5.2 Implementing Changes in Employment Status

A change in employment is effected by a personnel action generated by the Human Resources Department upon receipt of a written request from an authorized representative of a department (usually the administrative officer) telling of the intended change in an employee's status. Such forms or memoranda should include the old and new information, and the date on which such changes should become effective.

The following are examples of such changes in an employee's status. The chart below indicates the type of form or information which must be submitted by a department:

  • Personal Information Summary Form (for Administrative, Sponsored Research, and Academic Staff)
  • Invention and Proprietary Information Agreement, if required

Change in employee's classification or rate of pay

Memorandum (Job description required for promotion)

Transfer of an employee from one department, laboratory, or payroll to another

Notice of Resignation, Termination, Cancellation, Retirement, or Transfer Form (available from Human Resources Department)

Change in number of hours or percent of effort employee is regularly scheduled to work

Memorandum

Authorization of extended unpaid leave

Leave of Absence Authorization Form (available from Department of Human Resources)

Authorization of paid leave such as extended Sick leave

Memorandum

Return from leave (paid or unpaid)

Memorandum

Termination of an employee

Notice of Resignation, Termination, Cancellation, Retirement, or Transfer Form

Change in employee's personal address, name, phone, etc.

Any other circumstances requiring a change in an employee's status should be reported to the appropriate Human Resources Officer for the area.
